When you submit a contact or consultation request through this website, we collect:

• Your phone number

The callback form requests your name and phone number. The Ask Dr. B form requests your name and email address for an appointment request. It does not provide a free-text question field. Do not submit symptoms, treatment questions, detailed health information, or medical records through these forms. Website inquiries are not a substitute for the practice's Notice of Privacy Practices or patient communication channels.

How we use your information

Information submitted through this website is used only to:

• Respond to your inquiry or schedule a consultation
• Contact you about your appointment
• Review appointment and callback requests

We do not use your contact information for marketing without your consent. We do not sell, rent, or share your personal information with third parties for their marketing purposes.

How we store and protect your information

Form submissions are sent by email to the practice. For the callback form, the site also sends Freedman Automations a lead-log event containing the site identifier and form type, not the submitted name, phone number, or message. If the email send fails, the site sends an operational alert without the inquiry content. Do not use these forms to send detailed health information. Google Analytics loads only after you accept analytics cookies; you can change your choice using Cookie settings in the footer. Analytics is not configured for advertising.
